Simini Protect is a part of my surgical lavage regime I adopted it early and have never considered not using it. My surgical practice is dominated by orthopedic procedures especially the TPLO. I take all the appropriate measures to prevent infection and I believe the use of Simini Protect is a part of infection prevention. I added Simini protect to my practice despite my low infection rate. My goal is not a low infection rate it is zero infection rate. While this may not be an achievable goal, I will continue to work at it. Simini Protect is a safe and inexpensive addition, that literally takes no extra time. The addition of this product means a lot to my clients.
Our patients are not clean. They do not shower regularly, they do not wash in chlorhexidine prior to surgery they are dirty. They are increasingly plagued by allergic disease, both environmental and food related which leads to skin infections and ear infections. Many of our patients are treated with immune modulators and immunosuppressant drugs to control their allergies. They lick and chew at their incisions, because our clients and dogs hate e-collars. For these reasons alone I feel the addition of Simini protect was important to my practice. I can’t control the cleanliness of my patients but I can do everything possible to eliminate the biofilm and any bacteria that may be present. For minimal cost to the client we can add a layer of protection against infection.
This is the first product in veterinary medicine designed to protect against the acquisition of an intra-operative infection, work to lessen and eliminate the infection that already exists in fractures and revisions.

I have practiced at a referral center for 20 years and the last 4 years in mobile surgery with a caseload that is predominantly orthopedic. Study Design:
Patients were selected because they were at a high risk of infection or were revision cases, where an infection was present.
For each surgery, there was a swab to culture for bacteria just after the opening of the surgery. The procedure was then performed, and then just prior to suturing, the surgical wound was lavaged with Simini Protect Lavage. Following this lavage, a second swab was taken for analysis.
The patient was sutured, and follow-up occurred for all patients 6 months following the study.

Study Design:
A clinical study to report the outcome of total hip replacement (THR) revision cases using Simini Protect Lavage in addition to standard antiseptic protocol.
All patients had a minimum of 1-year follow-up and culture and a sensitivity test pre- and post- Simini Lavage were required.
Results (9):
- Of the 36 dogs in the study, only one case, which experienced three revision surgeries, underwent explantation due to persistent, clinically evident infection.
- The remaining 35 of 36 dogs showed neither clinical or X-ray signs of infection over a long period of time.

Study Design:
A human clinical study was performed in 45 patients with a total knee replacement and a periprosthetic joint infection.
During the revision surgery, just prior to closing, a sample was taken from the surgical wound, the wound was lavaged with Simini Protect Lavage, and then a subsequent sample was taken thereafter.
These patients were then followed-up at 3 months post surgery which is consistent with W.H.O. guidelines.
Results (2):
- Application of the Simini Protect Lavage demonstrated a 99.98% removal of bacteria in the wound
- 90 Day post-surgery infection rate was observed to be 38% lower than historical controls

A murine model was used to investigate the cytotoxic effects of irrigation solutions including Simini Protect Lavage (aka Bactisure) on the healing capacity of full thickness wounds.
These examinations occurred at the acute phase (Day 3) and the delayed phase (Day 10).
Results (8):
- Simini Protect Lavage had no greater cytotoxic effect on the host tissue when compared to normal saline alone.
- Wound healing was not delayed or compromised when treated with Simini Protect Lavage compared to normal saline alone.
Conclusion (8):
- Single short-duration use of these commercially available antiseptic irrigation solutions appears to be safe in an uninfected wound.
